Health insurance is a social security system by which the government provides its citizens with insurance services for the purpose of promoting public health. Such services encompass disease prevention, assessment, medical care, rehabilitation concerning diseases and injuries, childbirth and health promotion.

Subscription

Workplace health insurance

  • A resident alien registered under the Immigration Control Act, Article 31 and working at a workplace that provides health insurance shall become a health insurance subscriber under the National Health Insurance Act, notwithstanding the National Health Insurance Act, Article 5. (Ref. The National Health Insurance Act, Article 109 (2); Enforcement Decree of the said Act, Article 76 (1) 2).
Regional health insurance

  • The following persons may be insured under regional health insurance under The National Health Insurance Act by submitting an application for subscription to the National Health Insurance Corporation: an alien who has filed for alien registration under Article 31 of the Immigration Control Act and who qualifies for a status of sojourn under The National Health Insurance Act, Enforcement Rules, Schedule 9 or a person who has reported on his/her place of residence in Korea under The Act on the Immigration and Legal Status of Overseas Koreans, Article 6, but does not fall under the category of those eligible for workplace health insurance and will certainly stay in the country for more than 3 months for the purpose of study at a domestic school or employment. (The National Health Insurance Act, Enforcement Decree, Article 76 (2); The National Health Insurance Act, Enforcement Rules, Article 61 (2).

※ Essential professional persons engaging in a management or production/technology sector of a foreign-invested business under The Foreign Investment Promotion Act are qualified for D-8 visa (Foreign Invested Company). The spouse and unmarried children aged less than 20 of those qualified for the D-8 visa qualify for the F-3 visa (Accompanying spouse)

The time of acquiring status of sojourn and how to subscribe to health insurance

Employee Insurance (Workplace Health Insurance)

  • An alien acquires the status relating to workplace health insurance from the day he/she is employed by a workplace that provides health insurance [Guidelines for Application of Health Insurance for Korean Nationals Residing in a Foreign Country for a Long Term or Aliens, Article 3 (1) (Ministry of Health and Welfare Notice No. 2012-110 implemented on 9/1/2012) ].
  • When an alien has acquired the status of workplace health insurance corresponding to the National Health Insurance Act, the Enforcement Decree, Article 76 (1), the Employer should submit the report on the relevant acquisition of status (using the form stipulated in the  within 14 days of the acquisition the National Health Insurance Act, the Enforcement Rules, Schedule 6), along with a copy of the alien registration certificate or a certificate certifying such registration, to the National Health Insurance Corporation. Self-Employed Insurance (Community Health Insurance).

  • An alien who wishes to acquire the status of regional health insurance should submit the report on the acquisition thereof (using the form stipulated in The National Health Insurance Act, the Enforcement Rules, Schedule 5), along with a copy of the alien registration certificate or a certificate certifying such registration, and the necessary documents, depending on the status of sojourn (as per what is stipulated in The National Health Insurance Act, the Enforcement Rules, Schedule 10), to the National Health Insurance Corporation. In cases where the National Health Insurance Corporation can ascertain the facts associated with the report on the place of residence in Korea, alien registration, and health insurance premium imposition, etc. with the materials provided by the State under The National Health Insurance Act, Article 96, the relevant documents do not have to be submitted. (The National Health Insurance Act, the Enforcement Rules, Article 61(3)).


Criteria for imposition of health insurance premium and how to collect it

  • As regards the imposition and collection of a health insurance premium for a person subscribing to workplace health insurance, The National Health Insurance Act, Articles 69 through 86 apply [Guidelines on Application for Health Insurance for Korean Nationals Residing in a Foreign Country for a Long Term or Aliens, Article 6(1)1 (Ministry of Health and Welfare Notice No. 2012-110 implemented on 9/1/2012)]. The monthly health insurance premium for a person subscribing to workplace health insurance is as follows [The National Health Insurance Act, Article 69 (4)]:


Remuneration-based monthly health insurance premium

  • This amount is calculated based on the monthly pay received from the employer. An amount of monthly pay smaller than 280,000 is set as 280,000 won. An amount of monthly pay larger than 78.1 million won is set as 78.1 million won. [The National Health Insurance Act, Articles 69(4)1 and 70(1); The National Health Insurance Act, the Enforcement Decree, Article 32)].
  • The remuneration-based monthly health insurance premium is calculated by multiplying the monthly pay by the insurance premium rate (i.e. 5.8%). Concerning a person engaging in a business outside the country, the insurance premium is set as a half of that of those who subscribe to domestic workplace health insurance. [The National Health Insurance Act, Article 73 (1) and (2); The National Health Insurance Act, the Enforcement Decree, Article 44(1)].


Income-based monthly health insurance premium

  • When the income of a person subscribing to domestic workplace health insurance exceeds 72 million won per year, except for the remuneration included in the calculation of the remuneration-based monthly health insurance premium, the income-based monthly health insurance premium is calculated for the income except for the aforesaid remuneration. In this case, if the monthly income exceeds 78.1 million won, the monthly income amount is set at 78.1 million won. [The National Health Insurance Act, Articles 69(4)2 and 71(1); The National Health Insurance Act, the Enforcement Decree, Article 41 (2) and (4)].
  • The amount is reached by multiplying one half of the rate of the remuneration-based monthly health insurance premium by the subscriber’s monthly income. [The National Health Insurance Act, Article 69(4)2].


How to collect the premium from regional health insurance subscribers

The criteria for the  imposition of a premium on regional health insurance subscribers are as follows: [Guidelines on the Application of Health Insurance for Korean Nationals Residing in a Foreign Country for a Long Term or Aliens, Article 6 (2) (Ministry of Health and Welfare Notice No. 2012-110)]

  • Concerning an alien (a holder of a D-8 visa/corporate investor) whose income (wage) is ascertained, his health insurance premium is calculated by multiplying his/her monthly pay by the insurance premium rate for workplace health insurance subscribers. As for persons whose income (wage) is not ascertained or whose insurance, when calculated based on income (wage), is smaller than the household average insurance premium imposed on regional health insurance subscribers at the end of the preceding year, the insurance premium is calculated based on the following 2.
  • With regard to the health insurance premium for a subscriber (a holder of an F-3 visa/accompanying spouse) who has no income (wage) or whose income (wage) is hard to ascertain, the health insurance premium is set at an amount equal to the household average insurance premium imposed on regional health insurance subscribers at the end of the preceding year.
  • As for the health insurance premium for an alien permanently residing in Korea (a holder of an F-5 visa/permanent resident), it is set based on the same criteria as those imposed on Korean regional health insurance subscribers.


Collection of the premium from regional health insurance subscribers

  • A regional health insurance subscriber should pay the monthly insurance premium by the 25th of the preceding month based on the invoice. In the event of the addition of a further insurance premium due to the acquisition of status, the amount will be added to the insurance premium invoice.   • As for an alien permanently residing in Korea who has acquired the status of a regional health insurance subscriber, the insurance premium is imposed and collected according to the same criteria as those applied to Korean regional health insurance subscribers.
